Why might some domains not be found?

Not all domains can be discovered through reverse lookups. Sites using CDNs like Cloudflare hide their origin IP. Additionally, some hosting providers don't configure reverse DNS records, and external databases may not have indexed every domain on a given IP.

Does a reverse IP lookup show all domains on a server?

Not necessarily. The results depend on the data sources available. DNS reverse records only show domains with PTR records configured, and external databases index domains over time. Virtual hosts, recent additions, and sites behind proxies may not appear in results.
